Abstract: Normothermic Regional Perfusion-controlled donation after circulatory death (NRP-cDCD) is an advancement in organ transplantation with the potential to expand the number of organs for transplantation. The in situ reperfusion of organs post-mortem minimizes ischemic injury and enhances organ viability. However, ethical concerns arise regarding the intentional interruption of blood flow to the brain and the need for transparency in disclosing procedural details. The absence of standardized guidelines for disclosure exacerbates these ethical dilemmas, leading to varying approaches to disclosure and informed consent. Some argue for a "right not to know" specific clinical details, suggesting that the information is overly burdensome. Others advocate for full disclosure to uphold autonomy and informed decision-making. Religious and moral beliefs may conflict with NRP procedures, necessitating transparent discussions to allow for objections on moral or religious grounds. Promoting transparency respects potential donors' dignity and intrinsic value, ensuring they are not merely treated as a means to an end but respected as ends in themselves. Transparency is essential in fostering public trust in organ donation. Public engagement and education are pivotal in addressing diverse attitudes and beliefs and aligning organ donation policies with societal values. Transparency in NRP-cDCD promotes humanity by safeguarding the dignity of potential donors and upholding public trust in organ donation.
